Overview Dublin Springs is a 72 -bed behavioral hospital located in Dublin, Ohio. As a Discharge Coordinator, you will be a member of the clinical services team at Dublin Springs. Our team drives a "Clinical First" attitude to help fulfill our mission of "Changing People's Lives". The Discharge team collaborates with other members of the healthcare team to develop a plan for patients who are in need of ongoing psychological care following discharge. Discharge Planners play a vital role to our patient centric approach to delivering superior outcomes. Position Details Provides aftercare coordination for patients who need ongoing medical/psychosocial care following discharge Creates and maintains up to date aftercare coordination community resources Displays active involvement in treatment planning process, by setting up the aftercare appointments as identified by the patients' provider and therapist. Provides resource information to patients and families as appropriate for future care needs. Actively communicates with clients, families, aftercare providers, and referral sources. Demonstrates proactive communication with those involved with the patients' treatment. Creates a detailed Discharge Summary and Contract for each patient as assigned. Works well with the interdisciplinary team including physicians, utilization review and nursing staff members. Demonstrates active communication with team members. Completes all hospital and client documentation thoroughly, timely, and in accordance with hospital standards. Demonstrates appropriate crisis intervention and de-escalation skills. Acts as member of team when crisis occurs, assisting others to ensure patient safety. Displays a thorough knowledge of confidentiality. Completes releases appropriately. Demonstrates knowledge of HIPAA regulations and abides by those standards. Demonstrates a working knowledge of all clinical regulations, including TJC, Medicaid, and state and federal laws. Qualifications Requirements: Bachelor's degree in social work, counseling, psychology, or relevant state licensure or at least three years of clinical experience within an inpatient behavioral health setting. Prior experience with psychiatric and chemical dependency patients. Unencumbered clinical license if eligible/needed per state regulations preferred.
