With the creation of our new umbrella organization we now have our own identity with a unique payroll, benefits, performance management system, service recognition programs and other common practices across the system. _Position Summary:_ The Homecare Intake Specialist is the entry point for referrals into the Home Health service line. This position is responsible for processing physician service requests from hospitals, skilled nursing facilities, and physician offices. The Homecare Intake Specialist is also responsible for benefit verification for Certified and Hospice patients to ensure department and hospital financial goals are achieved through demonstrating critical decision making, analyzing insurance eligibility, analyzing payer conflicts, and ensuring benefit verification is on file. Homecare Intake Specialists must meet daily, weekly and monthly productivity standards. Knowledge of computer technology and the Internet (MS Office, Outlook). Including the proficiency in EPIC EMR. Home health intake experience is preferred. This job level requires comprehension of essential job responsibilities and continuous training to remain proficient with changes in payer requirements and the registration process. Ability to work remotely from home as needed per business needs. _Position Responsibilities:_ Key Areas of Responsibility · Problem solving skills. · Ability to communicate clearly with internal/external referral sources, clinical teams, colleagues, and insurance payers. · HIPPA compliance. · Document per policy and procedure within regulatory requirements in addition to Information Security policies and procedures. · Develop organizational skills, flexibility and time management skills and practices to accomplish daily tasking. · Actively engages and contributes to meetings and organizational initiatives. · Ability to multi-task and work simultaneously in multiple platforms. Referral Processing/ Financial Clearance · Proficiency using EMR and processing referrals from Epic work queues, Electronic Referral Platforms, and OnBase. · Utilizes patient identifiers to correctly create or select patient medical record, obtain accurate financial data. · Verify patient’s insurance and benefits information for home health services. · Accurate coordination of benefits. · Provides input on action plans to mitigate patient identification errors.* · Receptive to feedback on quality and productivity standards *Training and Development · Continuous education on industry standards. · Completes HealthStream learnings as required. · Complete initial 6 months of training for comprehension of essential job responsibilities and continuous training to remain proficient with front end enhancements to the registration process. · Develop proficiencies in other areas of registration in order to provide additional support and coverage Homecare Intake Specialist · Exhibits excellent customer service skills at all times.* · Knowledge of medical terminology.* · Attention to detail, multi-tasking and adaptive to change.* · Utilizes various on-line eligibility products and/or calling payor for accurate benefit information.* · Continuously improves own performance through HealthStream training, coaching, and responds to feedback provided through the quality monitoring tool and productivity reporting.* · Meets performance standards and productivity goals* · Knowledge and understanding of coordination of benefits. _Working Relationships:_ This Job Reports To (Job Title):Manager of Patient Access Education · High school diploma/GED equivalent Experience · Minimum:6 months in healthcare · Preferred:6 months in benefits verification and/or patient registration Language Skills · Excellent verbal and written communication skills _We take great care of careers._ With locations around the state, Hartford HealthCare offers exciting opportunities for career development and growth.
