Job Details The Social Worker coordinates and facilitates access to services and partners with community resources in order to achieve patients’ quality care. The ultimate goal is to foster the individual’s independence in living a healthy productive life in the community. Services are for primarily for adults. This position requires client outreach in-person on our mobile clinical outreach units and via text, phone and email. *Must be bilingual, English/Spanish. Summary of Duties: Coordinates care for clients identified through our community outreach (mobile units and Healthy Sundays), parishes, direct physician referrals and other sources. Assesses client’s health, behavioral and social determinant needs and links individual to treatment/resources as indicated. Creates care management plans that address clients’ needs and collaborates with internal and external resources. Advocates on client’s behalf where indicated to ensure their needs are met by other providers. Promotes clients’ self-management and empowerment. Maintains a working knowledge of payer requirements and develops relationships with insurance case managers. Adheres to organizational policies and procedures. Follows applicable federal/state regulations such as confidentiality and HIPAA. Interacts in professional, respectful and helpful manner. Maintains professional growth and development. Utilizes computer applications to track daily workflow and document on clients’ progress. Qualifications, Skills and Knowledge Required: Must be bilingual, English/Spanish. Graduation from an accredited university with a Bachelor’s or Master’s in Social Work 2-5 years of experience in a clinical setting Computer literacy, including Microsoft Office, data entry, retrieval and report generation. Experience with Electronic Health Records applications. A team player who can follow protocols to achieve a common goal. Excellent oral and written communication skills. Confidence to outreach to other health care organizations and personnel. Ability to self-motivate, prioritize, and be willing to change processes in order to reach a common goal. Sensitive to cultural diversity and low literacy issues. Important Skills: Fluent in Spanish, experience with immigrants, adult and child populations. Compassion, empathy, listening skills, organizational skills, people skills, problem-solving skills and time-management skills. Salary Range USD $69,500.00 - USD $95,000.00 /Yr. This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, or other forms of compensation that may be applicable to this job and it does not include the value of benefits.
